    78yo WM admitted to the hospital 9 days ago for a coronary artery bypass graft. Diagnosed post-op with a perioperative R MCA stroke requiring prolonged ventilator support for 4 days.

    HTN, CAD, COPD, PTSD

  • Social History: Married 50 years. Two adult children live 2-3 hours away and visit about every other month. Enjoys playing cards on Friday evenings with friends from the service. Primarily responsible for managing the family finances.

    Prior level of function: Independent with ADL, driving, and financial management PTA but hired help for home maintenance (e.g., yardwork and repairs) due to endurance.
