    67yo WM admitted to hospital 1 month ago with severe shortness of breath and cyanosis. Diagnosed with acute respiratory failure (ARF) secondary to COPD. Intubated for mechanical ventilation on Day 1. After 3 weeks, d/c to skilled nursing facility.

    Past medical/surgical history:
    COPD, HTN, anxiety, obstructive sleep apnea, recurrent respiratory infections, + smoker 1 PPD
