    70yo WM admitted to the hospital 4 months ago with c/o R-sided weakness. Diagnosed with L CVA. After 2 weeks in acute care, d/c to inpatient rehabilitation for 6 weeks. D/c from inpatient rehabilitation 2 months ago and began outpatient therapy services.
