    79yo WM admitted to the hospital 20 days ago with c/o left-sided weakness, numbness, and persistent L lean. Diagnosed with R CVA. D/c 2 weeks ago to inpatient rehabilitation center.

    Dementia, chronic abdominal pain with unknown etiology

    Environmental factors: Lives with wife in one-story house with 3 STE, no handrails. Walk-in shower (2” ledge). No DME. 2 adult children and 6 grandchildren, several of whom are available locally for support. Christian faith is important to him and his wife and they have strong social support from their local church.
