    50yo Black female admitted to hospital 2 weeks ago with abnormal swelling, bone pain, and uncontrollable fever. Diagnosed with osteomyelitis. D/c to skilled nursing facility after 12 days in acute care.

    End-stage renal disease, anxiety, type II diabetes

    Employment/work/school: College education. Former deli manager at supermarket, but had to quit due to decline in health.

    Play/leisure/interests: Used to be very active in local church, volunteering to cook Sunday brunches and to lead bible studies. Less active the past few years due to poor health, but still attends church every Sunday.

    Environmental factors: Lives with adult daughter in one-story house with 2 STE and portable ramp. Tub/Shower combo with shower curtain, handheld shower head, grab bars and tub transfer bench with back. Drop-arm BSC.
