    50yo Black female admitted to hospital 2 weeks ago with abnormal swelling, bone pain, and uncontrollable fever. Diagnosed with osteomyelitis. D/c to skilled nursing facility after 12 days in acute care.

    End-stage renal disease, anxiety, type II diabetes
