    Prior level of function: Modified independent for toileting, bathing, dressing, cooking, household tasks, and grocery shopping. Drove independently. Ambulated short distances within homes. When in community, walked short distances with FWW and frequent rest breaks. Utilized electric mobility vehicles while in stores.

    Employment/work/school: High school education. Retired textile factory technician.

    Play/leisure/interests: Enjoys cross stitch, reading, and attending weekly book club at the senior center.

    Environmental factors: Lives alone in two-story home with no STE, 13 steps to second floor. Primary living spaces, bedroom, and bathroom are on first floor. Walk-in shower (2” ledge), shower curtain, handheld shower head, shower chair, grab bars, and BSC. Has supportive neighbors and daughter who lives 10 min away and visits 2-3x/week.
