  • History of present illness:
    58yo WF brought to ER when found unresponsive in the morning by her husband. She was last seen well at 10pm night before. On presentation she was noted flaccid on the right side, found to have a right hemorrhagic stroke and immediately intubated. Pt was intubated and unresponsive. Husband at bedside.

    Past medical history:
    History of low back pain. A few months prior to stroke, she was found to be slightly lethargic and went to PCP who referred her to a cardiologist. Echo at that time indicated the diagnosis of A-Fib. Recently started on a beta blocker.

    Environmental factors: Lives in her home of over 20 years with husband and family. Husband is the primary caregiver and will care for Cheryl. Daughter is an OTA.

  • CT brain: Large right intraparenchymal hemorrhage with surrounding vasogenic edema. Small areas of subarachnoid hemorrhage in the right temporal lobe in the high convexity of the L parietal lobe. Multiple areas of acute infarct noted noted within portions of the right frontoparietal region, right basal ganglia, right frontal lobe, mesial temporal lobes and brainstem.
