    77yo WF admitted to the hospital 2 months ago with c/o L-sided weakness and dizziness. Diagnosed with L CVA and L visual field cut. D/c to skilled nursing facility after 2 weeks in acute care. D/c to home with HHOT after 6 weeks in skilled nursing facility.

    HTN, anxiety, arrhythmia, R hip osteoarthritis
