    47 yo WM admitted to the hospital 6 months ago with c/o R-sided weakness and slurred speech. Diagnosed with L CVA. D/c to inpatient rehabilitation after 5 days in acute care. D/c to home health therapy after 2 weeks in inpatient rehabilitation.
