    62 yo WM admitted to the hospital 4 months ago with c/o L-sided hemiplegia. Diagnosed with R CVA. D/c to inpatient rehabilitation after 1 week in acute care. D/c to outpatient therapy after 1 week in inpatient rehabilitation. Sustained mechanical fall 2 months ago at home which has led to increase in L shoulder pain.
