  • History of present illness:
    24yo WM with ASIA B SCI (T12, sensory incomplete) secondary to spinal infarct during combined liver-intestinal transplant at 5 yo. Hospitalized for 3 months in acute care followed by 1 month of pediatric inpatient rehabilitation. Received OT/PT throughout childhood. Does not currently receive therapy services.

    Past medical/surgical history:
    Short bowel syndrome with bowel lengthening surgery, combined liver-intestinal transplant. Hospitalizations for urinary tract infections.

  • Marital status: Single.

    Current level of function: Modified independent, completing ADL/IADLs at wheelchair-level with adaptive equipment PRN, including driving with hand controls and ultralightweight manual wheelchair.

    Employment/work/school: College education. Graduate student in occupational therapy doctoral program.

    Play/leisure/interests: Enjoys participating in adaptive sports program activities, including wheelchair basketball, bowling, boccia, pickleball, archery, and trap shooting. Takes guitar lessons and hangs out with friends weekly.

    Environmental factors: Lives with grandparents in one-story house with ramp at entry. Zero-entry shower with shower chair, handheld shower head, no grab bars.
